This is the fourth report on serious adverse events published by the Health Quality & Safety Commission, covering events reported by New Zealand's 20 district health boards (DHBs) and other providers, and the seventh since national reporting began.
Serious adverse events (previous referred to as serious and sentinel events) are events which have generally resulted in harm to patients. This report details serious adverse events in DHBs in the year 2012 to 2013.
